  1. Why are you leaving your job with us?
  2. Was there a single event that caused you to leave?  If so, please tell us about it.
  3. Do you feel like your job duties were clearly defined?
  4. If not, please define your job duties now that you have the experience to do so.
  5. Did your job turn out to be what you expected it to be? Please explain.
  6. What characteristics should we look for in your replacement?
  7. Do you feel like you received adequate feedback on your performance here?
  8. If you could have tweaked any part of your job to make things more effective what would you have  tweaked?
  9. Who here do you feel is one of the most valuable employees that work here?  Why?  What makes them stand out?
  10. What do you think makes a great manager?
  11. What can we do to improve our management style and skill?
  12. Do you have any other comments or suggestions for us?  Feel free to speak openly.  We want to become a better establishment and your insight is valuable to us in doing so.
